You Can Advertise in Healthcare Without Violating HIPAA

Discover how to leverage Google's offline conversions to feed Smart Bidding algorithms—without ever exposing Protected Health Information (PHI).

HIPAA-Compliant Google Ads for A Therapy Based Client

Marketing a clinical practice presents a unique challenge: Google Ads need complete conversion data to optimize campaign spend, but Google Ads is not HIPAA-compliant out of the box.

If standard ad tracking tags fire on a mental health website, they can accidentally send Protected Health Information (PHI)—such as IP addresses, page URLs indicating therapy topics, or client details—to Google.

We solved this challenge by pairing server-side data sanitization with Google's Multi-Source Conversion framework.

The Core Problem

  • Smart Bidding needs data: Google’s algorithms optimize toward conversions. Without clear outcome signals, campaigns underbid or misspend budget.

  • HIPAA strictly prohibits PHI leaks: Transmitting patient intake data or condition-specific URLs to third parties without a Business Associate Agreement (BAA) creates severe legal exposure.

  • Google won't sign a BAA for Ads: Google Ads tags cannot touch raw patient data.

The 4 Step Solution

Step 1: Target Active Intent, Not Audiences

Instead of retargeting past visitors (which violates health ad policies), we focused entirely on high-intent Google Search keywords (e.g., “licensed therapy near me” or “couples counseling”). Search ads responded directly to what users type, without building sensitive profiles.

Step 2: Strip PHI at the Server Layer

Before any conversion data reaches Google, it passes through a server-side container. This gateway automatically strips away:

  • Client names, emails, and phone numbers

  • Patient IP addresses

  • Clinical path names (e.g., converting /anxiety-treatment-booking into a generic /thank-you event)

Step 3: Capture the Click ID in the CRM

When a user submits an inquiry, we capture an anonymous (pseudonymous) Google Click ID (GCLID) and store it in the Therapy Practice's marketing CRM alongside the request timestamp. The patient’s clinical records remain separate in a secure EHR (Electronic Health Record) system.

Step 4: Import Backend Conversions Safely

Instead of relying on browser pixels, we import verified milestones (e.g., “Consultation Booked”) into Google Ads via Google Data Manager.

  1. Test safely: We start imports using Google’s 14-day diagnostic mode to verify data cleanliness.

  2. Feed the algorithm: Once confirmed, Google's Smart Bidding optimizes for real, booked intake calls—not spam form fills.

Takeaways:

You don't have to choose between strict HIPAA compliance and high-performing ad algorithms. By separating clinical records from attribution IDs and sanitizing conversion data before export, healthcare practices can safely leverage Smart Bidding to scale intake.
